# 1 How to ensure your plans are heard with Clair Fisher
In this introductory episode I talk to Clair Fisher. Clair talks about how her Advance Care Plan is helping her prioritise quality of life, the challenges she had in a recent hospital admission in ensuring her wishes were heard and the wellbeing advantages of planning ahead. Clair shares the strategies and tools she uses to document her wishes and what matters most to her.
Key messages from Clair
Listen, patients do want to communicate something of themselves.
Take that extra minute to show you have listened.
This is important, I want to know you are making plans aligned to my wishes.
Acknowledge and honour the work and effort that someone puts into making an Advance Care Plan.
The form isn’t enough, it’s the process, ongoing conversations, connections that are more important to outcome. Do that in the open so everyone knows what is happening.
It’s about the network of people around you being on the same page.
Advance Care Planning is a well-being intervention.
