In this blog I am proud to share the bespoke Advance Care Planning service I offer. Based on the principles of Getting Your House in Order, I work with clients exploring all elements of Advance Care Planning to create a unique and personalised document
Getting Your House in Order
Using elements of Getting Your House in Order, the model I have created to include all components of Advance care Planning, means that the service works well for any person at any stage of life or health. I work with clients who value having plans in place not because of any specific ill health, but because they are aware that life can change in an instant. I work with clients who are living with a deteriorating health condition and have struggled to join up the dots with Advance care Planning. I also work with clients who are in the last stages of life to ensure end of life planning is in place and they can feel assured that what matters most to them is documented and shared with the right teams and people.

Advance care Planning with Wendy Michell MBE
Confidentiality means I don’t usually share names or details of who I work with, but one very famous lady has shared publicly her Advance Care Planning journey, the late Wendy Mitchell MBE. Wendy was diagnosed with young onset dementia and reached out with me as part of research for her final book One Last Thing: How to live with the end in mind. How the service works
I usually meet a client three or four times to talk through the things that are important to them. Meetings can be face to face or on-line. We talk about What Matters Most, what quality of life means, and who and what is important to them. Things like organ donation, bucket lists and digital legacy are all covered.
From discussion a unique plan is created. There will be opportunity to include:
· A Statement of Wishes and Preferences
· An Advance Decision to refuse treatment
· Details of Lasting Power of Attorney and any wishes regarding CPR
· Legacy planning and memory making
· Plans for the last stages of life that map to the area a person lives
At the end of our discussions, information is recorded in the bespoke Advance Care Plan given to each client. We also discuss who to share the plan with.
